Academic Forms

Students are welcome to contact a CSS academic advisor for guidance with the completion of academic forms. Students should refer to the Mānoa and/or Outreach academic calendar for related deadlines.

Document Submission

Students should securely submit academic documents to the ACCESS Advising Team via our:

Emails are not secure, and we strongly advise against emailing academic documents.

To be used to declare, change to, transfer-in, or drop a major within College of Social Sciences.

  • Download Declaration of Major form.
  • NOTE: Advisor signature is not necessary when dropping a major.

Completion Steps:

  1. Obtain major advisor signature.
  2. Submit via the Kuali Document Submission Form.

To be used when applying for multiple undergraduate majors and/or degrees.

  • Download UHM-3 form.
  • NOTE: When adding a second major, be sure to indicate primary major.

Completion Steps:

  1. Obtain major advisor signatures.
  2. Submit via the Kuali Document Submission Form.

To be used to add a minor.

  • Download Minor form.
  • NOTE: If multiple majors, minor forms are only filed with primary college.

Completion Steps:

  1. Obtain approval from the minor advisor.
  2. Submit via the Kuali Document Submission Form.

To be used when the registration on STAR GPS is not available.

Completion Steps:

  1. Obtain instructor approval.
  2. Submit via the Kuali Document Submission Form.

To be used when the registration on STAR GPS is not available.

NOTES:

  • If dropping a course during the “W” period, bring the completed form to the Office of the Registrar (QLC, Room 010) after obtaining instructor approval.
  • If dropping to 0 credits during the “W” period, contact CSS Academic Advising (cssadvcl@hawaii.edu) for assistance.

Completion Steps:

To be used to increase the maximum number of credits.

  • NOTE: The current maximum is 19 credits per semester.

Minimum criteria for credit overload approval:

  • Recent semester of 18 or more credits taken with a semester GPA of 3.0 or higher without any incomplete grades issued or course withdrawals.
  • A petition is needed for an overload request of more than 24 credits.

To be used when a student does not enroll in a particular semester(s).

To be used only when a student attends a foreign institution.
